Conference attendance form not for Diplomates

The Conference attendance form used to be available for Diplomates. And some did submit them to the ECC. But Diplomates do not need to submit this form. It is now part of the Diplomate recertification form.

Diplomates can no longer add this form.

Those Diplomates who already used the form will keep it at their list of submitted forms, but can not add new ones.
